In the early 19th century, Dr. Noboru Yasumoto is summoned to a rural medical clinic in Koishikawa, where he is expected to start work immediately. The clinic, led by the stern Dr. Kyojô Niide, nicknamed "Red Beard", treats the poor and disadvantaged. Yasumoto is surprised and dismayed, having expected a position at a shogunate. He believes Red Beard wants access to his professional notes or that a female acquaintance's father is trying to get rid of him. As he gets to know the clinic's patients, including 12-year-old Otoyo, a former prostitute, Yasumoto may gain a new perspective on life as a doctor.
