In Spain, a former Nazi doctor named Klaus attempts to end his life, but survives with a paralyzed body dependent on an iron lung. His wife hires a nurse, and Klaus requests a specific teenager, Angelo, who befriends his daughter Rena. It is revealed that Klaus was a pedophile who derived pleasure from the fear of death in young boys before abusing and killing them. Meanwhile, Angelo is a disturbed individual who was a victim of Klaus' experiments and seeks to emulate his actions, as described in his diary.
