Anna's life is turned upside down when her war-veteran brother Bobby returns home, only to be institutionalized due to his mental health struggles. As her parents deny his existence, Anna's own grip on reality begins to slip. After Bobby's untimely death, Anna becomes increasingly unstable, and her tumultuous relationship with her gangster boyfriend Billy further exacerbates her mental state. In a desperate bid to regain control, Anna considers undergoing a lobotomy.
